Through our “Contact Me” form, we collect: Name, Phone Number, Email Address, Date of Birth (DOB), Line of Business requested, and Consent for Contact. This information is used solely to provide insurance services, updates, and marketing directly from our agency as requested.

As an independent agent, I am appointed with Farmers Insurance® as well as other insurance carriers. To provide you with the most competitive and accurate coverage options, we may share the information you provide with:

• Farmers Insurance® and its affiliates.
• Other third-party insurance carriers to whom we are appointed, for the sole purpose of obtaining insurance quotes and underwriting coverage at your request.
• Mobile information will not be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

By submitting your information and checking the consent box, you provide express written consent for Alexis Cozzi or a member of her team to contact you regarding your inquiry. We provide personalized insurance consulting, quote generation, policy management, and renewal services for auto, home, life, and business insurance.
